On this tab, typically used for a root cause analysis, you can add multiple root causes. Use this tab if multiple root causes exist and therefore the Record View, which can only record one, has no space. The fields on this tab are like those in the Root Cause Work Order Details section of the Record View, so again you will find all the work order closing codes. Additionally, the tab offers the following fields:

  1. Note—The note field is a required entry on this tab and should be used to identify the different root cause records on this tab.

  2. Contribution %—If there are multiple root causes you can use this field to record the calculated or estimated contribution of each to the failure handled by the case. For example, the pump failed because the impeller mounting nut was worn which contributed 75%, but the casing bolts were also loosened by vibration which attributed to the failure but only for 25%. Note that at insert the system will calculate the percentage remaining to 100 and default that value, but the system will not verify that the total adds up to 100 when you save records.

  3. Keep Synchronized with Case—If you select this checkbox, then changes on this record will also be copied to the work order closing code fields on the case header (see the Root Cause Work Order Details section). Only one root cause record can be selected at a time for this purpose.

  4. Root Cause Category—When you define multiple root causes you can use this field to categorize or group them, For example, man, machine, or process-related root causes.

  5. Why—There are 2 why fields on this tab.

  6. And there is a complete set of user defined fields available.

    Buttons:

    1. Copy from Case—This button will create a new record on this tab automatically based on the data entered on the Record View of the case. The system will copy all the work order closing codes and will fill in the Note with the case Description and will set Keep Synchronized with Case to selected if no other root cause record exists with that check box already selected. The Copied from Case checkbox will also be selected. Note that once copied you can change all the data as required. You can also copy as many times as you want.

    2. Update Work Order—This button will update the work order referenced on the case header in the Work Order field with the corresponding values from the selected record. This includes the closing codes, Failure Mode, Symptom, Tactical Cause, Human Factor, Workmanship, Human Oversight, and Method of Detection. Since the results of the root cause analysis may be delayed and may come after the work order is already closed, this update will happen regardless of the work order status. The system will also populate the WO Updated and WO Date Updated fields on this tab.
