This window enables you to issue a new repair form or modify an existing one.
A repair form is the actual repair or work that has been approved by the maintenance supervisor or foreman. It indicates the repair date and describes the nature of the problem as well as the work group that is assigned to carry out the job. A repair form also includes, in a different section, the information that is filled in by the person who performed the work, the cause of the malfunction, and action taken, as well as the down and repair time.
You can issue a new repair form from the Maintenance module, launched from the Instrument Index module. This can be done by a repair supervisor with appropriate access rights. Note that the repair options are not available at this stage. Subsequently, this repair form can be opened from the Maintenance Activities Summary window where the Repair options can be filled out. A new repair form can also be created from the Work Request window when opening a work request from the Maintenance Activities Summary window. You can then open the newly created repair form from the Maintenance Activities Summary window.
Instrument tag
Displays the tag that you selected.
Criticality
Displays the tag's criticality ( Process Safety System, and so forth) is displayed if defined in the Associate Categories and Criticality dialog.
Typical instrument
Displays the typical instrument to which this repair form is assigned.
Calibration — Calibration required
Select this check box if the instrument also needs to be calibrated. Note that this option is available only if you have made the required calibration settings prior to opening this repair form.
System message
Displays the latest calibration date, or if you defined calibration settings but have not yet performed calibration, that no calibration information is available.
Repair (scheduled) — Created by
Displays the person that opened this repair form.
Creation date
Displays the date that the form was created.
Work status
Select the required work status from this list. If the required work status is not on the list, select Work Status on the Tables menu to open the Work Status dialog, where you can add a new item to the Work Status list or modify it as needed.
Work order number
Enter or modify the work order number as needed. Note that work orders are preceded by the R notation by default.
Work request reason
Select the required work request reason from this list. If the required work request reason is not on the list, select Work Request Reason on the Tables menu to open the Work Request Reason dialog, where you can add an item to the Work Request Reason list or modify it as required.
Work request title
Type the work request title or accept the displayed on as needed. This field is filled in automatically after selecting a typical instrument.
Maintenance staff (scheduled)
Select the work group to which this repair has been assigned. If the required workgroup is not on the list, select Assigned Workgroup on the Tables menu to open the Maintenance Staff dialog where you can add new items to the Assigned Work Group list or modify existing ones as required.
Work request number
(available if you accessed this window from the Work Request window) Displays the work request number that you set in the Work Request window.
Priority
Select the appropriate repair priority.
Scheduled for
Enter or modify the date for which this repair job was scheduled.
Problem description
Type the appropriate problem description.
Repair (actual)
The Repair options contain the actual work order information. Open the Repair Form window from the Maintenance module, (Repair) icon in the module toolbar. to make these options accessible.
Starting date
Enter or modify the repair start date.
Completion date
Enter or modify the repair completion date.
Repair time
Enter the number of hours it took to repair this breakdown.
Down time
Enter or modify the time the instrument has been down due to this breakdown.
Maintenance staff (actual)
Select the maintenance staff assigned to carry out this repair. If the required option is not on the list, select Maintenance Staff on the Tables menu to open the Maintenance Staff dialog.
Outcome
Select the required item that describes the effect of the current failure, for example partial or complete unit shutdown, and so forth. If the required outcome is not available in the list, select Outcome on the Tables menu to open the Outcome dialog where you can add an item to the Outcome list or modify it as needed.
Repair action
Select the value that describes what action has been taken to repair the failure. If the required repair action is not available in the list, select Repair Action on the Tables menu to open the Repair Action dialog that where you can add an item to the Repair action list or modify it as needed.
Failure reason
Select the malfunctioning instrument's failure reason. If the required failure reason is not available in the list, select Failure Reason on the Tables menu to open the Failure Reason dialog where you can add an item to the Failure Reason list or modify it as required.
