  1. Select the Operations Logbook scope in the j5 IndustraForm Designer.

  2. Right-click the connection node or the relevant area node and select Configure Default Form for Category.

  3. Select the EAM Work Requests category. The EAM Work Requests template appears in the IndustraForm editor.

    When using an older version of the j5 IndustraForm Designer...

    The EAM Work Requests template won't appear; the default Operations Logbook template appears instead. When this is the case, the EAM fields need to be added before the form can be made live.

    To add the EAM fields:

    1. Right-click the cell you want the EAM field, and select Place Default Field.

    2. Select the relevant field for the location.

    Do this for each EAM field type.

  4. Edit the template as required.

    • Do not remove the EAM fields from the form as they are required for the form design.

    • You can rearrange the EAM fields on the form (if required). To rearrange the fields, you can either:

      • Cut and paste the fields into their new locations, or

      • Remove the fields, right-click the new location, select Place Default Field, and then select the relevant field for the location.

    • When using an older version of the j5 IndustraForm Designer, the EAM Place Default Field options appear in all the Operations Logbook category forms. The EAM fields are required for the EAM category only. You can ignore the EAM default field options for the other Operations Logbook category forms.

    • The Equipment cell in the EAM Work Request form allows one asset only to be selected (the Single Asset Only checkbox for the Asset type is selected and cannot be cleared).

  5. Select Make Live.

After the template is live, an EAM Work Request add button appears above the j5 Operations Logbook list view.
