During the Cause Analysis phase, one or more causes of a near miss or incident may be identified and recorded in the system. The cause can be described, and Root Causes selected from a preconfigured list of causes. The form will then allocate the cause to the appropriate Health, Safety and Environmental (HSE) Element.
