The initial state for the Incident Management workflow. In the Preparing section, add in a Title, the Date of Incident, and select whether the incident is defined as an Incident or a Near Miss.

Incident

Near Miss

Required fields for an Incident include:

  • Description of Incident - a short description of the incident or near miss

  • Area - select from the options available

  • Category - select Near Miss or Incident

  • Immediate Actions Taken - enter the actions taken immediately after the incident occurred

  • Location of Incident - enter the exact location of the incident

  • Reported By (automatically displays the name of the logged in user, but can be changed)

All Roles must be assigned to continue.

Required fields for a Near Miss include:

  • Description of Incident - a short description of the incident or near miss

  • Category - select Near Miss or Incident

  • Immediate Actions Taken - enter the actions taken immediately after the incident occurred

  • Reported By (automatically displays the name of the logged in user, but can be changed)

Only the Incident Initiator role is required to continue.

Additional information can be added, but it is not essential to the progress of the incident.

Once all the required information is complete, use the ACTIONS pane to select either:

  • Save to continue logging the incident. The workflow state changes to Preparing-Drafting.

  • Discard to cancel the incident or near miss. No further action is required.
